  • You will need to provide one of the following proofs of compliance:
  • Proof of Insurance with a company authorized to write in Louisiana, which is a letter of no lapse for the date or dates listed on the notice.
  • Proof of cancelled plate.
  • Proof of the date the vehicle was disposed by submitting one of the following containing the VIN:
    • Bill of Sale/Trade-in Documents/Lease Return
    • Total Loss Statement from the Insurance Company
    • Proof of Repossession from the Lien Holder
    • Junked or Salvage Receipt
    • Out-of-State Registration

If the vehicle is not insured or disposed of in a timely manner, then you will also owe a fee. Fees are based on the number of days without insurance and vary in each case.

Mobility Impaired Hang Tags

To obtain a hang tag for a mobility-impaired person, the following must be submitted to the OMV:

  • A Physician’s Certification of Mobility Impairment form (for handicap placards and license plates), DPSMV 1966, which has been completed by the mobility impaired individual’s physician.
  • A fee of $26.00 (includes state fee + convenience fee).
  • If the mobility impaired person has a Louisiana ID card/driver’s license with a clearly identifying photo and their Social Security Number is on file, additional identification is not required. Otherwise, the applicant must provide the same identification documentation as when obtaining an ID card/driver’s license. See below in the Driver’s License section for a list of acceptable documents.
  • Your current hang tag
  • Your mobility impaired identification card
  • A Physician’s Certification of Mobility Impairment form (for handicap placards and license plates), DPSMV 1966, which has been completed by the mobility impaired individual’s physician ONLY if you were issued a temporary hang tag. Only one renewal will be allowed for a temporary impairment.
  • Fee: $3.00
Mobility Impaired Plates

Individuals who have a permanent, lifetime disability can apply for a mobility-impaired license plate. Only vehicles titled in Louisiana will be eligible for issuance of mobility impaired Plates.

  • A Vehicle Application form, DPSMV 1799, which has been completed by the vehicle owner.
  • A Physician’s Certification of Mobility Impairment form (for handicap placards and license plates), DPSMV 1966, which has been completed by the mobility impaired individual’s physician.
  • If the vehicle is titled in the mobility impaired person’s name and is currently registered, the license plate and registration must be submitted.
  • If the plate is being used on a vehicle that is not in the name of the mobility impaired individual, the Designation of Mobility Impaired License Plate form (DPSMV 4222) must be completed and submitted.
  • Proof of insurance
  • If there is no license plate assigned to the vehicle at the time application for a mobility impaired plate is made, a copy of each vehicle owner’s current and valid photo identification (driver’s license or identification card issued by this state or another state) must be submitted.
  • A fee of $33.00 (includes state fee + convenience fee)

First-Time Driver’s License:

Every new driver must obtain a Temporary Instructional Permit (TIP) from a State OMV office, take Driver’s Education, and a road skills test at the OMV or with a third-party provider prior to obtaining a Driver’s License regardless of age. (Click here for a list of third-party providers)

Applicants who are 16 years old are eligible for an Intermediate License (click for details) after having a learner’s permit for 180 days. Once an applicant with a learner’s permit reaches the age of 17, he or she is eligible for a full license. Applicants over 17 years of age are not required to apply for a permit or intermediate license.

Bring your TIP, a sealed envelope containing the results of the road skills test, and proof of insurance if you own a car. If you are under 18 years of age, bring your birth certificate and your parent or legal guardian with their ID.

License Renewal:

If you have had a Louisiana license before all you need to renew is your license number or your social security number. You will also be required to pass an eye exam, so be sure to bring your glasses or wear your contact lenses.

Duplicate Licenses:

Be sure to bring your proof of insurance if you own a car and be ready to take another picture (if you are under 18 years of age, bring your Certificate of Enrollment from school, birth certificate, and your parent or legal guardian with their ID).

Motorcycle Endorsements:

An applicant must possess a valid Louisiana driver’s license or learner’s permit. An applicant must provide an original certificate of completion of the Department of Public Safety, Motorcycle Safety, Awareness and Operators Training Program to Brothers.

Public Tag Agencies are not allowed to add the MC endorsement if testing is handled by any other entity at this time.

Blood Type Indicator:

Autism Indicator:

Eligible Louisiana Office of Motor Vehicles customers can now request to have an “Autism” indicator added to their driver’s license. Qualified applicants include any person diagnosed with autism spectrum disorder by a qualified medical or mental health professional. The autism indicator will appear under the customer’s picture on the license.
The autism indicator is optional and can be removed from a credential at any time, as requested by the applicant. The “Autism” designation can be displayed on any class driver’s license, including the TIP, but may not be placed on Class I (Identification Card) or Class H (Handicap Identification Card) credentials. Applicable fees are required for driver’s license processing if adding or removing the Autism indicator.
Applicants must provide one of the following documents to the Louisiana Office of Motor Vehicles in order to have the designation added:
  • Medical Examiner’s Certification of Autism Spectrum Disorder form completed by the medical or mental health professional
  • A statement from a qualified mental health professional licensed in Louisiana or any other state or territory of the United States verifying the applicant’s disability.
Address Changes:

  • Utility bill for services installed at the applicant’s residential address (i.e., water, sewer, gas, electric, cable/satellite TV, internet, telephone/cell phone, or garbage collection).
  • Financial statement (i.e., bank/credit union account, investment account, credit card account, or loan/credit financing).
  • Applicant’s unexpired Louisiana driver’s license, permit or identification card that shows his current principal residence.
  • Employer verification, including, but not limited to, one of the following:
    • Paycheck or paycheck stub.

    • Letter from your employer on company letterhead.

    • W-2 for earnings issued.

    • Military orders issued.

  • A Louisiana driver’s license, permit or identification card issued to the applicant’s parent, guardian, spouse or other immediate family member residing in the same household. The immediate family member must be present and provide his Louisiana driver’s license, permit, or identification card in person.

  • Health insurance statement or explanation of benefits (EOB) for claim or a health care bill/invoice.

  • State of Louisiana or Federal income tax return or refund check.

  • Social Security documentation including Social Security Annual Statement, Numerical Identification System record, or Social Security check.

  • School record or transcript, report card, or student loan application.

  • Homeowners insurance policy or premium bill.

    Mortgage, payment coupon, deed, escrow statement or property tax bill.

  • Voter Registration card.

    Auto-Insurance Policy with applicant’s name and address.

  • I-797A issued by the United States Citizenship and Immigration Services.

  • I-797C issued by the United States Citizenship and Immigration Services.

    Refugee Address Verification Affidavit (DS-20).

  • Current, valid, rental contract/agreement and/or rent payment receipts (includes rental agreement/leases for a home, apartment, mobile home, dorm, extended stay motel, retirement/assisted living home, or a letter from a shelter)
